3. Raffle entry may vary depending on the model unit purchased. Promo applies to the participating models. See ANNEX A. 
4. The raffle promo is open to all that are currently residing in the Philippines.
5. This promo is valid in Philips flagship stores and authorized partners nationwide as indicated in Annex B. 
6. The list of qualified entries will be collated using an electronic raffle.
7. To officially join the raffle promo, customer must register by scanning the QR Code or visiting our website at evniamonitorspromo.com and fill out the form with the following details:

• Full Name (must be the same on the receipt)
• Contact Number
• E-mail address
• Preferred Delivery Address
• Store Name
• Purchase Date (must be within the promo period)
• Model Name
• Qty. purchased (if customer purchased more than 2 units)
• Product Serial Number
• Upload all the required attachments:

• Proof of purchase (must be clear and readable)
• Valid government issued ID
• Photo of the product serial number (can be found at the back or at the bottom of the actual monitor)

(If the purchase was made under a Company or Organization, an authorized representative must provide a Company ID and authorization letter signed by the company/organization’s signatory and send it to marketing@mmdsg.com.ph)

8. Customer will receive a confirmation email for the successful entry submitted.
9. Customer can register until July 30, 2024 at 11:59PM
10. This promotion is not valid in conjunction with other promotions or discounts and cannot be converted to cash.
11. Philips Monitor reserves the right to disapprove entries with incomplete requirements if the customer is unable to meet the conditions set out in this promo mechanics.
12. Entry will be deemed void if forged, manipulated or tampered in any way.
13. Customer’s name can only be drawn once.
14. The raffle draw and claiming of grand prize will be held at Vespa Philippines Motoitalia located at 324, 1504 Ortigas Ave, San Juan, Metro Manila.

14.1 If the grand prize winner is residing outside Metro Manila, shipping shall be covered by both organizers.

14.2 The grand prize winner will be contacted by one of the staff under Philips Monitor Philippines to confirm the delivery address.

14.3 The grand prize winner shall provide a clear photo together with VESPA S125CC upon receipt and shall be sent to Philips Monitor PH Official Facebook Page facebook.com/philipsmonitorsPH by the grand prize winner.

15. Raffle draw schedule: Tuesday, August 13, 2024 at 11:00AM
16. Consolation prizes will be sent to the winner’s registered address via courier, free of charge.
17. The raffle draw will be witnessed by a representative from the Department of Trade and Industry (DTI) together with the employees of Philips Monitor Philippines and Vespa Philippines Motoitalia.
18. Winners should claim the prize within 60 days upon receipt of official notice from their registered mail and/or email. Otherwise, the prize will be forfeited in favor of Philips Monitor Philippines upon the approval of DTI.
19. If the grand prize winner is unable to claim personally, he/she may send a representative and submit the following requirements:

1. Letter signed by the original winner stating that he/she authorizes the representative to claim the prize for or on behalf of the winner.
2. Two (2) original government issued IDs (with picture) of the grand prize winner.
3. Two (2) original government issued IDs (with picture) of the authorized representative.

20. Winners will be notified via phone call, their registered mail and/or email in no particular order.
21. By participating in this promo, the customer agrees to use his/her personal information for posting on Philips Monitor PH Facebook and Instagram page for the announcement of winners.
22. The tax prize of 20% shall be shouldered by Philips Monitor.
23. Redemption of the grand prize shall be 60 days from receipt of notification.
24. The grand prize is transferable to another person of the winner’s choice regardless of the relationship. The original grand prize winner is responsible for informing Philips Monitors and VESPA Philippines Motoitalia about the new recipient’s details.
